Staying Healthy

We all understand that ‘Prevention Is Better than Cure’ and all the staff at South Park Medical Practice are committed to promoting healthier living. In England heart disease is one of the major causes of premature disability and death; our health promotion efforts focus on reducing the risk factors for heart disease in our patients.

Risk factors include: smoking, obesity, lack of exercise, diet, high blood pressure, diabetes and high cholesterol.
Through the availability of health check-ups with our Nurses, and advice when you visit our Doctors, we aim to help you keep yourself healthy.

Our Nurses run special Health Promotion Clinics, if you are interested in living a healthier lifestyle please ask reception staff for details.
Health Promotion Clinics

These clinics are held on Tuesday and Thursday mornings 9:30 – 12.30 to monitor patients suffering from Asthma, Chronic Heart Disease, Diabetes and COPD. Additional Stop Smoking Service advice and support clinics are availabe by appointment.
Influenza Vaccinations

We offer free influenza vaccinations commencing in October each year to all our patients aged 65 and over, as wel as those particularly at risk due to heart disease, chest problems, diabetes, kidney disease and other health related conditions.
Pneumococcal Vaccinations

Everybody aged 65 and over should now be immunised to help protect them against pneumococcal infection which can cause diseases such as pneumonia, septicaemia (blood poisoning) and meningitis.
We offer free pneumococcal vaccinations to all patients over the age of 65, please ask at reception if you would like to book an appointment.
New Patient Medicals

We ask all newly registered patients over the age of 5 to attend a full health check with our Practice Nurse. During this appointment the Nurse may also offer advice on how to live a healthy lifestyle as well as an assessment of health risk factors.
Family Planning

South Park Medical Practice provides a full range of family planning services, which includes counselling and contraceptive advice and is available by appointment with our doctors and nurses during normal surgery hours.
Well Person, Heart Disease Prevention and Risk Assessment

This Clinic, for patients between the ages of 40 and 70, is run by our Practice Nurses with a Doctor on hand and includes cholesterol screening, a blood pressure check and advice regarding healthy lifestyles. This appointment also provides patients with a full print out of their results.
Baby Clinic

Dr Catherine Duncombe is approved to carry out child health developmental and postnatal checks. These baby immunisation and vaccination clinics are held every Wednesday 9:30 – 10.30

Immunisations are carried out by the Practice Nurse between 09.00 – 11.30 every Wednesday
General Health Checks for The Over 75s

This service is available for all our elderly patients and is arranged by appointment with one of our Practice Nurses.
NHS Health Checks

The NHS Health Check is available for all our 40 – 74 year old patients who do not already have a Cardiovascular Disease related condition. Everyone in this age group who is eligible is entitled to an NHS Health Check once every five years. If you are interested in this then please contact our Nurses or ask for a leaflet at reception.
Travel Clinic

We offer a full range of immunisations for those travelling abroad. Please contact the Surgery at least six weeks before travelling to collect a form to complete OR you may download the form from our documents page. When completed, please hand in the form at the surgery and an appropriate immunisation schedule will then be arranged for you with one of our Practice Nurses. A charge may be made for certain vaccines and malaria control; please ask at reception for details and to make an appointment with the Nurse.
